COVID-19 VACCINE
As of April 19, 2021, the City of Chicago is in phase 2 of the vaccine rollout. All Chicago residents 16+ are eligible for COVID-19 vaccines. People in phases 1a, 1b, and 1c remain eligible. While the list of those eligible expands, vaccine supply and appointments may be limited.
You can register for a vaccine with your primary care doctor, health clinic or hospital where you have gotten medical care in the past, or at Walgreens, Mariano's, Walmart, Jewel Osco and some Chicago Costco Pharmacies. Each pharmacy has a different process for sign up and registration, so it is best to reach out to them individually.
Below is a list of facilities in/around the 1st Ward that are offering vaccines and their appointment process.
WHAT PHASE IS THE CITY CURRENTLY IN? AM I ELIGIBLE?

ERIE FAMILY HEALTH CENTERS
HUMBOLDT PARK HEALTH VACCINE REGISTRATION
Humboldt Park Health is now offering the Pfizer COVID-19 vaccine to all Chicagoans 16 years of age and older, no appointment needed.
To get your vaccine: Walk into Humboldt Park Health at 1044 N Francisco Ave – 1st Floor Monday through Friday between 9AM - 4PM or Saturday between 9AM- 2PM.
Spanish interpreters will be available on site and language line will be available for all other languages.
No social security security number is needed. Bring an insurance card if you have one, but vaccines will also be provided for free without one.
AMITA Health St. Mary and Elizabeth
AMITA Health St. Mary and Elizabeth is offering Pfizer, Moderna, and Johnson & Johnson vaccines to those 18 and older. To schedule your appointment, call 773-661-1285 or click here.
ARMITAGE PHARMACY VACCINE REGISTRATION
Armitage Pharmacy is now offering Moderna vaccines to individuals 18 and older. To view open appointments and to book an appointment, please click here. Click on the different dates in the calendar section to view available appointments. Open appointments are released periodically.
UNITED CENTER VACCINE SITE REGISTRATION
Anyone over 16 living in the City of Chicago is eligible for an appointment at the United Center. There are no zip code restrictions.
Eligible residents can access an appointment in two ways:
- Online: go to zocdoc.com/vaccine to register for an appointment. If you cannot access the website...
- By Phone: call (312) 746-4835 to reach the multilingual call center. This hotline will be open Monday – Saturday 8:00am – 8:00pm and Sunday 8:00am – 4:00pm.
Given the anticipated high demand, residents with capacity to access appointments online should do so online in order to leave the call center available for those without access to the online platform. For more information and updates, please visit the City's United Center webpage here.
UI HEALTH VACCINE REGISTRATION
UI Health is now offering the Pfizer COVID-19 vaccines at Credit Union 1 (525 S Racine) Monday - Friday from 8 am to 5:30 pm. All Chicagoans over age 16 are eligible. To book your appointment, please call 833-536-9829 or book online here.
VACCINE OPPORTUNITY FOR VETERANS
The SAVE LIVES Act allows Jesse Brown VA Medical Center to provide the COVID-19 vaccines to all Veterans regardless of their age, health conditions, or nature of discharge from military service. Veterans’ spouses, caregivers, widows, and widowers can also get the vaccine at Jesse Brown.
Veterans who currently get their health care from VA can make their vaccine appointment by calling (312) 569-5801 or by contacting their care team. All other Veterans, spouses, and caregivers can sign up to get the COVID-19 vaccine by completing a short online form at www.va.gov/health-care/covid-19-vaccine/stay-informed. This form registers Veterans, spouses, caregivers, widows, and widowers to receive the vaccine. People who register using this tool will be contacted by Jesse Brown to schedule their appointment, or they can call (312) 569-5801 to schedule their appointment 24 hours after completing the form.
Spouses and caregivers do not need to register with or schedule a vaccine appointment with a Veteran. Registration and scheduling are done individually.
Vaccine appointments are available Monday through Saturday with some evening appointments available. Vaccines are administered at 2215 W. Harrison Street in Chicago. Some vaccine appointments are available at other Jesse Brown community-based outpatient clinics.

VACCINE OPPORTUNITY FOR UNION WORKERS
Union workers who live or work in the city of Chicago, hold a current union card or be a union retiree, and qualify under any of the eligibility criteria in Phase 1a, 1b or 1c as defined by the Chicago Department of Public Health (CDPH) are eligible to get a COVID-19 vaccine through the Chicago Federation of Labor. For more information, union members should visit here. Those interested in receiving a vaccine must have a valid appointment to be vaccinated and union cards will be checked at the door for access to the site.
CITY OF CHICAGO IN HOME VACCINATION PROCESS
The City of Chicago is working to integrate mobile home health care vaccination services to reach home-bound individuals who are unable to go to the City’s COVID-19 vaccination sites to get the vaccine. Please note that City vaccination sites are ADA accessible, and this program is specifically for those who are unable to leave their home for medical reasons.
To be eligible for this program you must:
- Be a senior OR a person with a disability who requires in-home assistance OR
- Have to use adaptive equipment (like a ventilator, crutches, a walker, a wheelchair, etc.) and/or accessible transportation to leave home AND,
- Leaving home is not an option for you because doing so requires considerable and taxing effort
If you fit these criteria, please complete the contact form so the City can coordinate an in-home vaccination with you or the person in-need.
CITY SITE: WRIGHT COLLEGE
The City-managed vaccination site at Wright College, located at 4300 N. Narragansett, will begin to offer appointments to the general public on Wednesday, April 28th (for Chicago residents). Beginning next week, at City-managed vaccination sites, we will be offering vaccines to individuals 16 years of age or older (under 18 requires a parent or guardian on site to consent).
